November symptom tracker

Hi, I’m Jess!

I’m Jess and I love to create symptom trackers that turn my good and bad days into little works of art!

Since I live with a rare migraine condition (NDPH), I track my daily migraine pain. I like to glue the tracker into a journal so that I can make daily notes on the page throughout the month to remind me what activities I did or didn’t do that might have affected my pain level that day or on following days.

As you can see, I rank my pain on 6 levels each day and then color in the spaces accordingly. You can use it however you wish!

Tip: if your pain varies throughout the day, you can use multiple colors in the space for that day.

At the end of each month, I have a picture that shows me at a glance how my month has been!

Sometimes I look at my picture and realize that the month wasn’t as bad as it seemed (a string of bad days can make a whole month feel horrible) and then other months, I look back and see what a strong, fierce warrior I am for persevering! Go me!!

YOU can use this image to track whatever symptoms you deal with daily. There is no right or wrong way to complete it!

    how can i give a packet about donating to someone i think my pain management dr may . i would like to give him some physical information and write a letter
